Keeps this file in sync with your plan files — adds missing cards, removes stale ones.
- Reads all ~/.claude/plans/*.md and extracts their H1 title
- Reads all ~/Desktop/*-plan.md files for the Stockpile section
- Adds cards for any plans not already listed
- Removes cards whose files no longer exist
- Reports: plans added / removed / total
